Starting kindergarten, entering middle school, or heading off to summer camp — each of these milestones requires a current physical examination. At Happy Days Pediatrics, we make the process simple and efficient. Dr. Barakah Day provides thorough school and camp physicals and completes all required Texas school forms and most camp health forms accurately and on time.
The most important thing to know about school and camp physicals: schedule early. Spring and summer are our busiest appointment periods, and families who wait until the last minute often find themselves scrambling for appointments. We recommend scheduling school physicals in the spring before the school year and camp physicals at least 4 to 6 weeks before camp begins.
If your child is due for a well-child visit around the same time as their school or camp physical, we can often combine the two into a single comprehensive appointment. Call us and we will figure out the most efficient approach for your child’s schedule.
What To Expect
- Complete physical examination
- Height, weight, BMI, blood pressure
- Vision and hearing screening
- Immunization review — updates administered if needed
- Completion of all school-required Texas health forms
- Completion of camp health forms and medication administration authorization
- Any required specialist clearance documentation if applicable

Kindergarten entry physical: schedule in spring before the school year
Annual school physical: schedule July through August
Camp physical: schedule 4–6 weeks before camp start date

What forms should I bring to the appointment?
Download and bring the specific physical form required by your child’s school or camp before the appointment. Most school districts post their required forms on their website. If you are unsure which form to use, call the school or camp directly.
Can I combine a school physical with a well-child visit?
In most cases, yes. A well-child visit is more comprehensive and can satisfy the school physical requirement in addition to covering everything else the wellness exam includes. Call us when you schedule and we will book the right appointment type.
Is a physical required every year in Texas?
Texas requires a physical exam for kindergarten entry. Beyond that, many schools and extracurricular programs have their own annual physical requirements. Check with your specific school or program for their current policy.
My child needs a physical form completed for camp, but camp starts in two weeks. Can you help?
