- Chronic Condition Management
A chronic condition in childhood is not just a diagnosis — it is something that shapes daily life for the child and the entire family. Managing it well requires more than occasional checkups. It requires a physician who knows your child deeply, stays current on the most effective treatment approaches, adjusts the plan as your child grows, and is available when things change. That is exactly the standard of care Happy Days Pediatrics provides for children with ongoing medical conditions.
Dr. Barakah Day manages a range of the most common pediatric chronic conditions within our practice, coordinating with pediatric specialists when the condition requires additional expertise. We serve as the central point of coordination for your child’s care — making sure every specialist, every treatment plan, and every medication is working together rather than in isolation.
Our goal for every child with a chronic condition is the same: optimal control, minimal disruption to daily life, and a child who grows up understanding their own health and how to manage it. We involve children in their care from the earliest age appropriate — because kids who understand their conditions grow into adults who manage them well.

FREQUENTLY ASKED QUESTIONS
Does my child need a specialist, or can you manage their condition?
Many of the most common pediatric chronic conditions — including asthma, eczema, allergic rhinitis, obesity, and constipation — can be effectively managed within our practice. Conditions that require subspecialty expertise (such as Type 1 diabetes, complex epilepsy, or oncology) will involve specialists, but we remain your child’s primary physician throughout.
How often will my child need chronic condition follow-up visits?
It depends on the condition and how well it is controlled. A child with mild, well-controlled asthma may need follow-up every 6 months. A child with poorly controlled asthma may need monthly check-ins until stability is achieved. We build a follow-up schedule that reflects your child’s actual needs.
Can you complete my child's school asthma action plan or 504 documentation?
Yes. We complete asthma action plans, food allergy action plans, medication administration forms, 504 plan supporting documentation, and any other school or camp medical forms your child needs.
My child's condition seems to be getting worse. Do I wait for the next appointment?
No. Call us. If your child’s chronic condition is showing signs of worsening — increased symptoms, medications not working as well, new triggers — do not wait for a scheduled visit. We will either bring them in, adjust their treatment plan by phone, or coordinate urgent specialist care. Waiting is not the right answer when a condition is actively changing.
